Health Requirements:

To ensure a healthy campus for all, Mount Saint Mary's University has health requirements that need to be completed by all full-time undergraduate and seminary students.

The following items are required using forms provided by Mount Saint Mary's University.  These forms are available at The Wellness Center or web2.msmary.edu/studentsandstaff/wellness

1.  Health History

2.  Physical Exam performed within the last year

3.  Immunization Records:  In accordance with guidelines established by the Advisory Committee on Immunization Practices (ACIP) and the American College Health Association (ACHA), the following immunizations re required.

A.  Measles/Mumps/Rubella (MMR):  Two doses for all students born after 1956.  The 1st dose must have been given after 1st birthday, the 2nd dose age 4-6 or later.

B.  Tetanus/Diptheria/Pertussis (DTP): Completed primary childhood series and tetanus booster within the past 10 years.

C.  Polio:  Completed primary series in childhood

D.  Varicella (chickenpox):  Either history of the disease or 2 doses of vaccine given 1 month apart.

E.  Hepatitis B Vaccine:  Series of 3 doses given at 0, 1 and 6 months.  If not completed prior to entry, must be initiated.

F.  Meningococcal (meningitis) vaccine:  1 dose given within the last 3-5 years (required of resident students only, see #4 below)

Lab work documenting immunity may be substituted for immunization records.  Copies of lab results must be attached to immunization record.

A medical contraindication must be documented by a health care provider.

If for religious/personal reasons, the student/guardian chooses not to receive vaccines, an exemption form must be signed. 

4.  Meningitis Vaccine Form:  In accordance with Maryland state law effective 2000, the University will provide the resident student (or parent/guardian if under age 18) with information about meningococcal disease and vaccine.  The student (or parent/guardian) may choose to receive or waive the vaccine after evaluating the information.  This form must be returned by all resident students indicating their decision prior to moving on campus. 

5.  Tuberculosis Screening:  Tuberculosis skin testing of all college students is not recommended (ACHA, CDC).  Skin testing is performed if indicated by risk assessment.  The screening questions are located on the immunization record.
